Okay, your second link is helpful. I think I see what the problem is. The first link assumes that any use of {{ref}} and {{note}} is to create footnotes, which has been superceded by {{reflist}} (or <references>). However, a number of places use those first two templates to create explanatory notes that are separate from the footnotes.
