  1. I don't see that the template is needed, civility issues are more-or-less covered by the uw-npa series or the uw-agf series. Even if such a template were needed, there are issues with the design: The green stop hand represents a mixed signal, the single issue format and the threat to "block without further notice" are not supported by the blocking policy.   COMMENT  Don't template the regulars is an essay and one can choose to ignore it, if one so chooses, notwithstanding the fallout that might occur.

On the other point, and assuming this is a final draft of the template, I agree with Guerillero this is not the right venue for discussion of the proposed template. Discussions about proposed templates should occur at WT:UTM so that all those who work on such templates (e.g., Wikipedia:WikiProject user warnings (shortcut → WP:UW)) can participate. The more participants, the greater the synthesis of ideas, and the stronger the consensus and buy-in with the final result.

However, if this is not the final draft and you are only seeking input, then it might be best not to use the Support, Oppose, Neutral format and just permit us to engage in the usual collegial, collective, talk page consensus building and re-writing before moving discussion of the final draft to WT:UTM.
