On lap 63, Teo Fabi attempted to lap Hiro Matsushita entering turn 3. The two cars touched wheels sending both into the outside wall just as Stefan Johansson and the leader, Paul Tracy were approaching from behind. Johansson successfully avoided the accident on the inside and Tracy braked and attempted to avoid the accident on the outside, but was caught with nowhere to go, was collected by Matsushita. The three cars came to rest in the outside of turn 4 and Tracy began to climb out of the car. Two other cars drove under the accident on the inside, but behind, a pack of two cars, one of whom was Arie Luyendyk, slowed as they approached the accident scene taking up the entire inside lane. Jacques Villeneuve, still running at high speed, came upon the accident scene quickly. Villeneuve jinked to the outside to avoid the slowing cars, but did not see the wrecked cars of Matsushita and Tracy until he was nearly upon them. With nowhere to go and no grip to stop the car as he was now in the 'grey' on the outside of turn 4, Villeneuve plowed head-on into the side of Mastushita's car, removing the engine and gearbox and knocking Tracy's car into the outside wall. In an interview with ESPN's Jon Beekhuis, Tracy recalled that, "Somebody's [Matsushita's] motor just clobbered me right in the side." Villeneuve's car continued to roll towards the inside of the turn 4 exit after the impact. Dominic Dobson tried to drive under Villeneuve's car, but ran out of room on the inside and interlocked wheels with Villeneuve's car, which launched him over Villeneuve's nose and back onto the track, breaking the front wing and suspension.

On lap ___ Nigel Mansell made his final pitstop. Wanting to avoid a stall, as he had done on his first stop, he hit the accelerator hard, causing massive rear wheelspin and lighting up both of his rear tires as he pulled out of the pits. However, this caused an imbalance in grip between his front and rear tires as the rear tires now had much more grip than the front tires as the rears were now much warmer. This caused Mansell to understeer out of the warm-up lane and onto the grass verge that separated the warm-up lane from the track. Mansell re-joined the track directly in front of traffic and was nearly hit from behind by Buddy Lazier (mistakenly identified by ESPN reporter Derek Daly as Scott Goodyear), who managed to avoid him on the inside.

On Lap 166, Mario Andretti crashed on the backstretch. As the field came upon the accident scene, John Paul Jr., Scott Goodyear, and Micha
