Ashley Smith was an inmate at Grand Valley Institution for Women in Kitchener, Ontario, Canada. She died on October 19, 2007 in an "apparent suicide" that generated significant controversy and the Ashley Smith inquest.

Her family raised objections to the inquest, and in the years following her death Canadian news media reported that prison officials had "tailored" reports of the use of force on Smith prior to her death.
