The 2018–19 CONCACAF Champions League play-off round was the final round of the 2018–19 CONCACAF Champions League qualification. Twelve teams competed in this round, four in the League Path and eight in the Champions Path. The series determined the final six participants in the CONCACAF Champions League group stage (the six winners) and the CONCACAF SuperLiga group stage (the six losers).

The first legs will be played on 21 and 22 August, and the second legs will be played on 28 and 29 August 2018.
