- 2015: implementing what seemed to be an uncontroversial template-protected edit request but wasn't [1]
- 2016: changing a single-template TfD, which I had participated in, into a merge discussion [2]
- 2016: sending a mass-message that reported to be on behalf of a wikiproject but wasn't [3][4]
- 2019: non-minor changes to portal with listed maintainers [5][6] (only one section, as part of response to help request posed to Wikiproject Portals instead of those maintainers, but still should have checked more carefully or at least demonstrated in user/sandbox space)
