This template is used for the table rows within the Declared monuments of Hong Kong article (for each declared monument).
Includes formatting, allowing the identifier, name, an image, declaration date, location, description, and references for each monument to easily show up in the article.
If no image currently exists for the monument, this template will also provide a suitably unsightly button, which, when clicked, will bring a user to the Wikimedia Commons upload page (autofilling everything but the image), making it very easy for someone who does have an image of the monument (an image which they should hopefully have permission to upload) to easily fix this problem by uploading that aforementioned image, before then editing the article to make this template link to that image (hopefully).

This template is mainly used on Declared Monuments of Hong Kong.
