Name change

I think the name of the article is better without the initial "The", or in other words: "Nuts & Bolts of PBM." It appears that the actual title of the publication was such and it was called that by other sources, although not in all cases, certainly. It should probably have a name and an alternate name noted. I'm not particularly hung up on what the name should be, although I would say examine what all the available sources say and kind of decide it by "consensus" almost - whatever comes up the most.
